Responsibilities: Accepting new referrals for home health services. Ensures seamless transition of care from hospitals to in home care. Verified insurance and patient copays. Review referrals and patient history to determine if patients qualify for home health services. Work directly with hospitals, SNF, and physician offices to obtain physician orders for home health services.
Requirements: Prior experience working as a Patient Service Representative, or Medical Front Office. Health Insurance knowledge - HMO, PPO, Medicare, Medi-Cal, and other federal and state agencyprograms. Strong professionalism and communication both in person and over the phone. Ability to pass on boarding requirements of both a background screening and drug test. Ability to build and foster relationships. Previous experience with customer service and interacting with patients and providers.
