The Healthcare Patient Services Coordinator is an integral part of the Clinical Patient Quality team. The Patient Services Coordinator collaborates with multiple disciplinary team members across the continuum of care. The Coordinator supports the team with reducing fragmentation of patient care, improves compliance and access to patient care, supports efforts to reduce or remove treatment barriers, and assists patients in navigating their patient care services. This position will involve telephonic management and direct patient contact through follow up at clinic appointments. Receive patient requests for assistance and refers patient to appropriate member of Care Coordination Care Team for resolution. Documents activities, plans, and results in an effective manner to maintain health record and provide reports. Consistently and positively communicates and collaborates with colleagues, supervisor, and customers (internal and external). Communicates collaboratively in situations involvingconflict so the conflict deescalates and is resolved. Maintains current knowledge of and consistently follows HIPAA requirements to insure privacy. Demonstrates adherence to Standards of Conduct. Completes and turns in standards of conduct acknowledgement card. QUALIFICATIONS/SKILLS: At least 2yrs as Medical Office Referral Coordinator, Medical Assistant or LVN experience. Strong problem solving and navigating skills to work through simple to complex patient inquiry. High School Diploma required, AA degree preferred. Must be available to start immediately.
